BACKGROUND
This federal agency in accordance with public law 107-67, has established a child care subsidy program to help assist qualified
familes obtain quality, affordable, and licensed child care services for children under age 13 or disable children under the age of 18.
ELIGIBILITY
The Employee must be a full or part-time active employee.
SUBSIDY AMOUNT
Income amount is based on the (combined with spouse, if applicable) latest family IRS 1040 adjust gross income (AGI).

NOTICE ABOUT TAXES
Employee's are advised to consult a tax advisor of any potential tax income reporting. Or if the program meets the criteria for exclusion of all or part of any income report based on 26 USC 129 requirements.
PARTICIPATION
Applications will be processed in the order they are received. Awards will be based on the amount of authorized funds available and the current anticipated benefits already awarded and obligated in the administrative year. Valid applications will receive a decision letter within 14 days.
AWARD PAYMENTS
For benefit payment the employee and provider will both sign the invoice and submit it to Lewis River Services for processing.
